Since life and disability insurance is primarily state-regulated, and we are licensed by state, we need your street address so that we may establish which carrier might apply. Your name, phone number and/or email address enable you to be contacted, per your request. The other information is for our internal use to service your request, so that we or our administrators may access your customer information. So that we can provide our products and services to you, we must collect personal information. The type of information we collect depends on the products and services you are interested in and whether you are the proposed insured. For example, we may collect your name, address, and telephone number. And we may collect other information such as your health, financial, and employment history.
